Ultrasound of the abdominal cavity for the presence of fluid

Ultrasound of the abdominal cavity for the presence of fluid (ascites sonography) is a quick, painless and very sensitive examination. It focuses on detecting free fluid in the abdominal cavity, which is normally present only in minimal amounts. At our clinic, we perform this examination as a separate diagnostic test or in combination with a comprehensive abdominal ultrasound. It is completely radiation-free, takes 15 to 30 minutes, and the doctor will discuss the results with you immediately. Ideal for quickly clarifying the causes of an enlarged abdomen or unexplained problems.

What is ascites and why does it occur

Ascites refers to the pathological accumulation of fluid in the abdominal cavity. In a normal state, the abdomen contains only a few milliliters of fluid, which ensures smooth movement of the intestines and organs and protects them from friction. When the balance between the formation and outflow of fluid is disturbed, its accumulation occurs. This is manifested by a gradual increase in the circumference of the abdomen, a feeling of heaviness or fullness and pressure on the diaphragm with subsequent shortness of breath when lying down or during physical exertion. In advanced cases, ascites can complicate breathing or movement.

The most common causes include liver disease, heart failure with right ventricular overload, abdominal tumors, inflammation of the peritoneum (peritonitis) or kidney disorders and low protein levels in the blood. Depending on the nature of the fluid, a distinction is made between transudate (with increased pressure in the vessels or low oncotic pressure) and exudate (with inflammation, infection or tumor).

Ultrasound is key to detecting even small amounts of fluid and allows not only to confirm the presence of ascites, but also to estimate its amount and nature. Early diagnosis significantly helps the patient's treatment and prevents serious complications.

What does an abdominal ultrasound focused on fluid reveal

Modern devices provide excellent imaging of free fluid in the abdomen. The doctor assesses its amount, location, nature and any other changes.

Possible findings include:

  • Minimal, moderate or massive ascites

  • Septa or clots in the fluid (in inflammation or bleeding)

  • Signs of portal hypertension or liver cirrhosis

  • Focus changes in the organs or on the peritoneum

  • Presence of blood or pus

When is an examination for the presence of fluid in the abdomen appropriate

We recommend an abdominal ultrasound for ascites primarily in the case of a visible or gradual increase in the abdominal circumference, a feeling of heaviness, fullness or tension in the abdomen that worsens after eating or lying down. Unexplained shortness of breath when lying on the back, swelling of the lower extremities or a general feeling of fatigue often occur. The examination is also appropriate in laboratory abnormalities such as reduced albumin, increased liver tests, coagulation disorders or electrolyte imbalance.

This examination plays a key role in suspected liver cirrhosis with portal hypertension, heart failure, abdominal tumors (ovarian carcinoma, pancreatic carcinoma, gastrointestinal tumors or peritoneal carcinomatosis), acute or chronic inflammation of the peritoneum or tuberculous peritonitis. It is also necessary for regular monitoring of chronic patients with a known risk of ascites, where it allows monitoring the development and effect of treatment.

Preventive sonography will be appreciated by everyone with chronic liver diseases (hepatitis, steatosis, cirrhosis), heart problems (cardiomyopathy, valvular defects), after intensive chemotherapy or abdominal surgeries, where there is a risk of secondary fluid accumulation. Regular check-ups help to detect ascites in the early stages, when it does not yet cause significant difficulties and allow for gentle intervention before complications occur. If you have any of the above risk factors, we recommend considering the examination - it often brings quick relief and a clear plan for further care.

Abdominal wall sonography

In addition to targeted examination for the presence of fluid in the abdomen, our clinic also offers a comprehensive abdominal wall ultrasound as a logical extension or independent diagnosis. This examination focuses on the soft tissues of the abdominal wall and is invaluable in the differential diagnosis of pain or protrusions in the abdominal area.

Abdominal wall sonography reliably detects umbilical or abdominal hernias, lipomas, cysts, inflammatory foci, hematomas after injuries, swelling or tumor changes in the soft tissues. It often explains the cause of unclear abdominal pain, a visible protrusion or a feeling of pressure that worsens with exertion, coughing or lifting heavy objects.

The examination is completely painless and does not require any special preparation. It only takes a few minutes and we will discuss the results with you immediately on the spot.

If you suspect a hernia, postoperative complications or unclear pain in the abdominal area, you can undergo both examinations (for fluid in the cavity and abdominal wall) directly in a row in one appointment.
